Camper Application Package

Instructions: Open the Camper Application, fill in the blanks then print the document out and mail to the camp with your deposit. There are many of the areas on the document that are mandatory to be filled in. All dates are in MM/DD/YY format. Print out the other four documents. Attach your deposit as shown on the Camp Dates-Fees page and mail all pages of the Camper Application and Camp Dates-Fees page to the address shown at the top. Do not mail the cover letter, emergency contact requirement and list of camper needs. These are for your information only. You will be mailed an invoice showing your deposit and balance due shortly after receipt which will confirm your reservation.

New for 2012, all pages of the Camper Application form and Camp Dates-Fees form must be initialed. Failure to initial each page will result in the forms being returned and possible loss of your camp date.
